Summer Camp 456 (going into 4th-6th grade in the fall)

June 29 – July 3, 2013
Cost $275
Please complete the 3 steps to be considered register for camp:
1) Complete Participation Form – Fax form to 480-963-4229, turn into staff in B200 or turn into church office
2) Make a Payment for camp
3) Please respond to UCYC email request for their Registration/Medical Form
All forms & payments must be completed by June 24th to attend camp.
Limited scholarship are available by filling out Scholarship Application.
